By registering for and/or participating in any course provided by Revive Health & Wellness, LLC, the participant acknowledges and agrees to the following Terms and Conditions:

Training is provided by Revive Health & Wellness, LLC, Nina S. Silva, BSN, RN, PHN, and/or aligned certified instructors utilizing current American Heart Association (AHA) Guidelines and/or American Red Cross (ARC) standards and instructor materials. Courses may include hands-on skills practice, simulations, videos, written examinations, and physical activities.

Completion of a course does not guarantee certification. Participants must successfully complete all required attendance, participation, skills testing, and written examination requirements established by the certifying organization.

Participant understands and acknowledges that CPR, AED, First Aid, BLS, ACLS, and PALS training involves physical activity that may include, but is not limited to:

• Kneeling 
• Repetitive chest compressions 
• Lifting or moving training equipment 
• Physical exertion 
• Exposure to latex or cleaning products 

Participant voluntarily assumes all risks associated with participation, including but not limited to physical injury, muscle strain, soreness, aggravation of existing medical conditions, illness, emotional distress, disability, or death.

Participant affirms they are physically and medically capable of participating in all required course activities and have consulted a healthcare provider if they have any medical concerns or limitations.

Participant understands that CPR and emergency response skills do not guarantee survival, recovery, prevention of injury, or any specific medical outcome. Outcomes depend on numerous factors beyond the control of Revive Health & Wellness, LLC, including but not limited to:

• Patient condition 
• Response time 
• Environment 
• Availability of emergency medical services 
• Individual medical complications 

To the fullest extent permitted by law, participant voluntarily releases, waives, discharges, and holds harmless Revive Health & Wellness, LLC, Nina S. Silva, all instructors, employees, contractors, affiliates, volunteers, landlords, facility owners, and representatives from any and all claims, liabilities, demands, damages, losses, costs, attorney fees, injuries, illnesses, disabilities, death, or causes of action arising out of or related to:

• Participation in training 
• Use or misuse of learned skills 
• Certification activities 
• Equipment use 
• Physical activity during class 
• Actions taken during real-life emergencies 

This release applies whether caused by negligence or otherwise, except in cases of gross negligence or willful misconduct as prohibited by California law.

Participant agrees to indemnify, defend, and hold harmless Revive Health & Wellness, LLC, Nina S. Silva, instructors, affiliates, employees, contractors, and representatives from any claims, lawsuits, damages, liabilities, or expenses arising from participant’s actions before, during, or after training.

Participant understands that Good Samaritan protections vary by state and circumstance. Revive Health & Wellness, LLC does not provide legal advice or guarantee legal protection related to rendering emergency care.

If liability is imposed despite this agreement, participant agrees that the total liability of Revive Health & Wellness, LLC and its representatives shall not exceed the total amount paid for the course.

Participants are expected to maintain professional and respectful behavior throughout training.

Revive Health & Wellness, LLC reserves the right to dismiss any participant without refund for:

• Disruptive behavior 
• Unsafe conduct 
• Harassment 
• Intoxication or impairment 
• Failure to follow instructor directions 
• Academic dishonesty or falsification of information 

All course payments are non-refundable.

Participants may request one reschedule within 30 days of the original course date, subject to availability. No-shows, late arrivals, or failure to complete the course may result in forfeiture of all fees.

Certification cards are issued electronically unless otherwise specified. Participants are responsible for verifying the accuracy of their name and contact information prior to card issuance.

Revive Health & Wellness, LLC is not responsible for delayed, denied, suspended, revoked, or replacement certifications caused by:

• Incorrect participant information 
• Failure to complete course requirements 
• Certifying organization delays 
• Violations of certifying body policies 

Participant grants permission for Revive Health & Wellness, LLC to use photographs, video recordings, or audio recordings taken during training for educational, training, marketing, promotional, and social media purposes unless written notice is provided before class begins.

All training materials, presentations, manuals, videos, forms, and course content are proprietary and protected by copyright laws. Unauthorized reproduction, recording, distribution, or resale is prohibited.

Training provided by Revive Health & Wellness, LLC is educational only and does not replace professional medical advice, diagnosis, treatment, or emergency medical services. In an actual emergency, participants should immediately activate EMS/911.

These Terms and Conditions shall be governed by the laws of the State of California. Any dispute arising from participation, registration, or services provided shall be resolved exclusively in the courts located in Contra Costa County, California.

If any provision of this agreement is found invalid or unenforceable, the remaining provisions shall remain in full force and effect.

By registering, signing electronically, checking an acknowledgment box, submitting payment, attending class, or participating in training, participant confirms that they:

• Have read and understood this agreement 
• Voluntarily agree to all terms and conditions 
• Understand the risks associated with participation 
• Waive certain legal rights 
• Consent to the use of electronic signatures and records 

I acknowledge that I have carefully read and fully understand the Revive Health & Wellness, LLC Terms and Conditions, Liability Waiver, Assumption of Risk, and Participant Agreement. I voluntarily agree to all stated terms and understand that participation in training involves inherent risks.
