Revive Health & Wellness, LLC CPR/First Aid/BLS/ACLS/PALS Training Terms and Conditions

Training is provided by Nina S. Silva, BSN, RN, PHN, and certified aligned instructors using American Heart Association (AHA) 2020 Guidelines and American Red Cross (ARC) Instructor’s Manual (Updated December 2021). All courses use official materials and certified feedback manikins.

• CPR and related skills do not guarantee saving a life; they increase survival chances only.
   
• Performing CPR may cause injuries such as broken ribs or internal trauma.
   
• Revive Health & Wellness, LLC, Nina S. Silva, and instructors are not liable for any injuries, damages, health outcomes, brain damage, or death resulting from use of skills outside training facilities.
   
• Outcomes depend on EMS response times, environment, and individual medical conditions beyond our control.
   
• Training does not replace professional medical advice; consult your provider regarding any health concerns about participation.
   

Individuals providing reasonable aid in good faith are protected from liability unless acting with gross negligence.

Participants agree to indemnify and hold harmless Revive Health & Wellness, LLC, Nina S. Silva, and aligned instructors from all claims, damages, or expenses arising from their actions during or after training.

Training and materials are provided “as is” without any express or implied warranty of merchantability or fitness for a particular purpose.

Maximum liability for Revive Health & Wellness, LLC and instructors is limited to the amount paid for the training course.

Participants affirm they are medically cleared and physically able to perform the activities required in training, including chest compressions and other physical skills.

This training does not replace professional emergency medical services. Participants must always call EMS immediately in an emergency.

No refunds will be issued for cancellations. Participants may reschedule their training session once within 30 days of the originally scheduled class date by contacting Revive Health & Wellness, LLC. No-shows or cancellations without timely notice forfeit registration fees.

Full attendance, participation, and skill demonstration are required for certification.

Participants consent to use of photos/videos for training or promotional purposes unless notified in writing prior to class.

Respectful behavior and safety compliance are required; disruptive participants may be dismissed without refund.

Notify us at least 72 hours before class to arrange accommodations for disabilities.

Training materials are proprietary and copyrighted; unauthorized copying or distribution is prohibited.

These Terms and Conditions are governed by the laws of the State of California. Any disputes arising will be resolved exclusively in the courts located in Contra Costa County, California.

Terms may be updated; continued participation signifies agreement to current terms.
